Puberphonia research

SFurther research in Puberphonia
I like to use the capsule endoscope, consists of a small, wireless, pill-sized camera that can be kept in the throat and allows direct visualization of the all the parts involved in speech.
The camera is fed through the patient’s nose and positioned just above the throat. Once the camera is in place, patients can talk. With real-time throat images from the camera, puberphonia boys can learn to control their throat’s effect on breathing. We can also monitor it.
This novel approach that involves visual biofeedback, training in specific breathing techniques, and feedback regarding performance. They can see the mechanical changes in the throat, almost all of these boys have thoughts during talking that are working against wish. Therapeutic laryngoscopy during talking creates an opportunity to address both mechanical issues and performance psychology issues,
Siva ENT hospital will dedicate exclusively to groundbreaking medical research and treatment of boys with puberphonia disorders.

Breath of fire

Breath of fire in puberphonia treatment.
Buberphonia boys is instructed in inhaling or “sniffing in” through the nose and exhaling gently through pursed lips.                      Attention is given to abdominal expansion during inhalation and abdominal relaxation during exhalation.
We give feedback, visual imaging techniques are taught as a method of keeping the vocal cords open during rapid breathing. These include visualizing the throat being as wide open during inhalation.
Control breathing include an open jaw and relaxed-tongue position while concentrating on diaphragmatic breathing.
These exercises are initially instituted in breathing, and finally during speaking.

Puberphonia, breath

Puberphonia/practice abdominal breathing
Clavicular / intercostal breathing create tension in neck and larynx. This contributes to laryngeal tightness and spasm. Diaphragmatic breathing :results in a relaxed shoulder girdle and abdominal expansion.

Puberphonia research

Speech is reliant on the powerful air flow that is supplied via our respiratory system. When we breath out,the sound filled air travels up from laryngopharynx to oropharynx, powerful air flow is created in the direction of the air flow to buccal cavity.
1.Uvula divide the air flow, right and left.
2.Nasal air also attracted towards the buccal air flow.
3.The divided air in the buccal cavity circulate and make a powerful air flow.
4.The mouth shapes the sound of our voices.

Puberphonia research

Our research
Tongue & uvula movement during speech:air that carries the sound: a real-time ultrasound evaluation of speech ... It appears that real-time ultrasound imaging of the oral and pharyngeal cavity is a potentially valuable technique for the clinical investigation of puberphonia.

Uvula & tongue are the speech organ

Puberphonia is a functional speech disorder due to the faulty (air carrying sounds) movementsus of uvula & tongue, characterized by the habitual use of a high-pitched voice after puberty, hence why many refer to the disorder as a habit disorder.

Friday, 22 November 2019

Function of uvula

Speech is created with pulmonary pressure provided by the lungs that generates sound by phonation through the glottis in the larynx that then the air filled with sounds is divided by uvula which the sole cause power of speech. The sound filled air enter on either  of the buccal cavity. It circulate and touches the different  parts of mouth and vibrate.Pushed out byhe vocal tract into different vowels and consonants.

Relive phobia in puberphonia

Puberphonia can be mild, and not cause much of a problem, or severe, when it becomes a serious communication disorder. The puberphonia can also vary for the same person. A person might find that they have periods of puberphonia, followed by times when they speak without a Puberphonia. It needs correction to live without phobia.

Life force for speech/puberphonia

Before we dive into how to cure puberphonia in your voice, let’s quickly explore what it actually is.

Resonance is really what we’re all striving for in our voice. It’s what we want.

It’s that deep full reverberating sound.

But don’t fall into the trap and think that volume equals resonance, because it doesn’t. Resonance from your voice is not forced. You don’t have to use extra air and risk cracking your voice which can happen if you try to speek too loud.

This is where the importance of puberphonia training the comes in. The sound created at vocal cords flot in the air and comes out from laryngopharynx to oropharynx. On it's way it is divided to right and left by uvula to increase it's strength.Automatically it attaches air from the nosopharynx. The divided forceful air circulate in the mouth. It gets the life.Anything moves,it is, speech is produced.(My, original finding, cytoplasm circulate and get life).

Your head and chest are your two registers and resonators. The third resonator is your pharynx.

Upset by puberphonia

The incidence of puberphonia in general population is more than we expect. The most common symptoms include pitch breaks, childish voice, hoarseness, breathiness, difficulty in vocal projection, and visible laryngeal muscle tension.
Puberphonia can be compared with iceberg example, because Puberphonia is an iceberg, iceberg means that only 10% of the problem is on the surface and the other 90% is hidden underwater like deep-rooted psychological & emotional disorder, and by the treatment of this it can be ccured.
Parents commonly neglect this disease, and they think that it will be automatically corrected, but in this, the boy suffers the most because their peers laugh at them, everyone makes fun of them, do mimicry of  them, and due to this they feel humiliated.
From their social life, their mind gets disturbed, their confidence gets disturbed, they have low self-esteem, they do not want to go in front of people, they don’t want to sit in a group of people, everyday they feel that there is something missing in them, if they want to say something they stop themselves because they think that if they speak they might be identified as puberphonia, people will laugh at them, others will make fun of me and won’t listen to me like this. Such boys have many psychological issues and they are emotionally disturbed due to puberphonia.
Now will talk about its implications, it doesn’t have any physical, but there are dangerous psychological & emotional implication of Puberphonia, A boy himself from inside suffocates, and he becomes helplessness, and they are unable to understand that what to do & what not.              Puberphonia is not just a speech disorder; it’s a serious communication problem, is not motivating throughout work, they don’t have the motivation to meet people,
Confusion, excessive thinking, nervousness, loneliness, infinity complex, introvert nature, low self – esteem, lack of confidence, fear of failure, fear of rejections, anxiety, tension, stress, sadness.

Uvula, the organ for speech

Good and comfortable speech.
Move one's hand from abdomen to head while speaking.
===============================================
Treatment for puberphonia. Dr.M.Kumaresan Muthiah MS:DLO
------------------------------------- Dr.Navin Bharath MS(ENT)

This intensive mode of administration is consistent with theories of motor learning and skill acquisition, as well as with principles of neural plasticity (the capacity of the nervous system to change in response to signals), and is critical to attaining optimal results. The treatment not only simulates the motor system but also incorporates sensory awareness training to help individuals with Puberphonia recognize that their voice is too soft, convincing them that the louder voice is within normal limits, and making them comfortable with their new louder voice.

1. Mouth must be made as a big cavity. It improves vocal loudness by stimulating the muscles of the voice box (larynx) and speech mechanism through a systematic hierarchy of exercises. Focused on a single goal “speak LOUD!” – the treatment improves respiratory, laryngeal and articulatory function to maximize speech intelligibility. The treatment does not train people for shouting or yelling; rather, full mouth uses loudness training to bring the voice to an improved, healthy vocal loudness with no strain.

2. Speak with whole body. When you speak move the hand from abdomen to the head. The sound travel with the hand. You can really feel it.

3. Speak with the movement of soft palate.

4. The ward should come out by the movement of tongue.

5. Flex the head and extend the dead while speaking.
6. Uvula divides the speech sounds comming from the larynx. This Air in the oral cavity circulate like a current ,vibrate, resononate and produce the desired speech.

Puberphonia in SIVA

ENTREPRENEURS AND PROFESSIONALS VOICE IS IMPORTANT BOSS

                    ENTREPRENEURS AND PROFESSIONALS
VOICE IS IMPORTANT BOSS
Conversation is one of the most basic forms of communication, but don’t underestimate it. A simple, conversation with your clients can build trust. An innocuous bit of small talk with a stranger can turn into a sales opportunity. Learn to speak in an approachable voice, confident way that you can apply to any situation.
The sound of our voice makes a strong impression. Here are the most important elements to keep in mind. Resonance:-Resonance is defined as “the quality in a sound of being deep and full most important of all, every voice projects the personality of its speaker. In other words, voice determines personality. We use our voice; it should have the force and frequency for people to listen.    Possibly, the raised voice used to listen others and modulation with adequate frequency helps others to understand our views.
A speech disorder is a condition in which a person has problems creating or forming the speech sounds needed to communicate with others. Some speech disorders can cause people to develop anxiety disorders. Stressful situations can trigger anxiety, resulting in more pronounced speech disorder symptoms.
Pathophysiology
There are three basic types of speech impairments: articulation disorders, fluency disorders, and voice disorders.
The most common speech disorders in adults
Stuttering: It may come as a surprise that many famous actors and entertainers experienced stuttering, including James Earl Jones.
Apraxia: If you have apraxia, your tongue and lips aren't able to move in the correct way to produce sounds.
Dysarthria:  Voice disorders involving the larynx (voice box). Dysarthria can occur when you have trouble moving the muscles of your lips, tongue, vocal folds, or diaphragm. It can result from degenerative muscle and motor conditions, such as multiple sclerosis, muscular dystrophy, cerebral palsy, Parkinson's disease or stroke.
Aphasia:  It is a communication disorder that results from damage or injury to language parts of the brain. It's more common in older adults, particularly those who have had a stroke. Aphasia gets in the way of a person's ability to use or understand.
Cluttering: It is a speech and communication disorder characterized by a rapid rate of speech, erratic rhythm, and poor syntax or grammar, making speech difficult to understand words. Aphasia does not impair the person's intelligence.
Dysprosody:  which may manifest as pseudo-foreign accent syndrome, refers to a disorder in which one or more of the prosodic functions are either compromised or eliminated completely. Prosody refers to the variations in melody, intonation, pauses, stresses, intensity, vocal quality, and accents of speech.
Verbigeration: It is obsessive repetition of random words. It is similar to perseveration, in which a person repeats words in response to a stimulus. However, verbigeration occurs when a person repeats words without a stimulus.
Tangential speech:  Tangentiality is a communication disorder in which the train of thought of the speaker wanders and shows a lack of focus, never returning to the initial topic of the conversation.
What is poverty of speech? In psychology, alogia (Greek á¼€-, “without”, and λόγος, “speech”), or poverty of speech, is a general lack of additional, unprompted content seen in normal speech. As a symptom, it is commonly seen in patients suffering from schizophrenia and schizotypal personality disorder, and is considered a negative symptom.
What is clanging speech? In psychology and psychiatry, clanging refers to a mode of speech characterized by association of words based upon sound rather than concepts. For example, this may include compulsive rhyming or alliteration without apparent logical connection between words.
Speech anxiety: Can range from a slight feeling of “nerves” to a nearly incapacitating fear. Some of the most common symptoms of speech anxiety are: shaking, sweating, butterflies in the stomach, dry mouth and rapid heartbeat. Pressured speech is when someone's speech is rapid and disorganized.
Pressured speech/rapid speech: It is not a disease or disorder by itself. Rather, it is a symptom of an underlying disorder. There are many potential causes of pressured speech, including schizophrenia, some forms of anxiety, and bipolar disorderly voice.
Puberphonia:  Puberphonia is a disorder predominant of post-pubescent male (without known organic cause), who exhibit higher register than other male of the same age group, known as "Juvenile Resonance Disorder" or “Little Girls’ Voice”. The high voice may be produced at the top of the chest register or in falsetto; this high voice may be associated with mild dysphonia and increased, effortful phonation. Vocal instability is often marked with extensive frequency swing like cutting of voice while speaking continuously. Vocal impairment in male is generally of higher pitch i.e. above 200Hz, and sometimes observable with downward pitch breaks. Downward pitch break reveals the natural lower pitch level of 110-125 Hz.  Possibly elevated than the average airflow, if breathy voice accompanies higher pitch.
There are a number of proposed causes for the development of this speech problem. The etiology of puberphonia can be both organic and psychogenic in nature. Causes of puberphonia include: 1. Emotional stress 2.Delayed development of secondary sex characteristics.  3. Resistance to pubertal changes 4. Self-consciousness resulting from an early breaking of the voice
More importantly, they are not coming out or converse or mingle with other person or society; because of they feel ashamed of their voice. After ADEM.S APLE formation they supposed to talk “BOSS VOICE rather talking with high frequency like female folk. As a result they cannot explain their view or even a truth of an occasion of explaining the real cause for any issues which could make them too ashamed of the situation. So they psychologically affected with depression. Besides, they cannot retain in their job longer period. The frequent quitting the job which leads to financial constrain in their family. Negative social reactions commonly occur and it overall affects social and vocational goals of them.  Actually they suffer in their all walks of their life, like physically, psychologically and economically.
The habitual use of a high pitch while speaking is associated with tense muscles surrounding the vocal folds. Assessment and treatment of puberphonia is usually conducted by an otolaryngologist (ENT). This voice is not a disorder that is likely to go away on its own. Without treatment, the changes in the patient's voice can become permanent. Treatment can involve direct voice therapy, indirect voice therapy, or audiovisual feedback. In most cases when traditional voice therapy is ineffective. In SIVA E N T Hospital Dr.M.Kumaresan and his team does voice care management successfully and  the result  is 95%,who come from far-away places for treatment can practice their voice care by watching  in the face book and you tube. In Siva  E N T Hospital Dr.M.Kumaresan does a small pharyngeal resonance manipulation, followed by immediately speech of fire training is given by trained team at least for three days bring more success in gaining male voice frequency and the success rate is > 95%.This training is given as suggested by Tholkappiyam         (தொல்காப்பியம்)
